In the event of a death in someone's family, a member of the family is eligible for three days off paid leave. This is known as compassionate leave.

Although it may vary depending on who the person is employed by, in general it is also possible, for those who are responsible for funeral arrangements, to be granted an extra four days off of unpaid leave. It could be that certain businesses would allow more time than this depending on particular circumstances such as the distance involved in travel for the funeral.

In terms of an anniversary of a bereavement, by law, employers are not required to grant compassionate leave. In this case it would usually be possible for holiday to be taken, which could be arranged ahead of time. It is of course possible that certain businesses will grant compassionate leave in these circumstances.

To write an application of leave to an employer, certain details would need to be included. Personal details would need to be included, such as the employee's name, address and what their particular job role is. The most important information would be the reason for requesting leave and the period of time off that is being sought. It may perhaps be appropriate in the application to state who the deceased is.

In conjunction with this, perhaps the most sensible thing to do is for an employee to speak to their line manager so that they can clearly explain the situation and express what they are requesting. This may be a more appropriate setting for the employee to express their feelings. In the case of a death anniversary this would likely be the best setting to express the importance of this event to a manager.
