Receipt of both earnings and/ or unemployment compensation has to be reported to the IRS at the end of each tax year. The required form 1099-G must be provided by the employer or agency which is responsible for the payments in question to the recipient by 31st January of each year.

A variety of agencies and employers frequently make these forms available before they are due. It is therefore a good idea to check the relevant Unemployment Compensation (UC) website at regular intervals before the actual cut-off date.

On the left hand side of this page is a list of quick links. Selecting the 'Reprint Tax Forms' option will open the page necessary for Weekly Claim Certification.

Once the claimant has entered his or her address, SSN and PIN, form 1099-G, showing the total UC payments received during the year and form UCO-49T, displaying any repayments to the agency following over-payments, will be available with an option to be printed.

Although address, SSN and PIN are sufficient information for most departments, some sites such as the Tennessee Dept of Labour & Workforce, will require additional information. This will include, for instance, the claimant's name, date of birth and the relevant tax reporting year.

It is also possible to request duplicates of the 1099-G statements for current or previous years. This can be done either by phone, on 1-800-204-2418, or via e-mail at [email protected].

Requests for duplicates must be accompanied by the claimant's name, their social security number and, if applicable, any change of address, as well as the specific year, this is the calendar year, for which the request is being made.

The requested duplicates will then be forwarded to the claimant by the agency.
