You can get hold of a W-2 and W-3 online through the SSA’s website: http://www.socialsecurity.gov/employer/
From there just create copies of your W-2 or W-3 to print off and fill in.
These are mainly for filing with the SSA (which you can do here: http://www.socialsecurity.gov/employer/) but you can also use these files to submit your W2 or W3 with your state authority or local government.
You might also want to use these templates for your employer, or simply to keep in your own personal file.
