This actually happened in a 1992 episode of Inspector Morse, which was shot in an Italian vineyard. Frances Barber was wearing a quietly modest top and the director thought it would look better if she wore it back-to-front. She said she didn't know how it exposed until she saw the segment played.
One unexpected outcome was that, after the episode was shown in Europe, she received a letter from an Eastern European military unit asking for photographs. They had adopted her as their pin-up.
